As a member of our Contact Center team, you will play an important role in helping people connect with the services and resources available to them. You will manage member relationships throughout their service journey, provide information about our programs and processes, and coordinate support services on behalf of our healthcare provider partners.

You will engage with members by phone and other communication channels, answer questions, help schedule services, coordinate referrals and follow-up, document interactions, and work with internal teams and healthcare partners to address member needs. This is a non-clinical role focused on communication, coordination, problem-solving, and delivering a positive member experience.

Community Outreach Specialist

As a Local Coordinator, you will help individuals and families navigate community resources and connect with programs that can support their health and well-being. You will build relationships within the communities we serve and connect people with local health resources, social service programs, and organizations that address social and practical needs.

You will work closely with members, families, community organizations, and healthcare partners to identify available resources, facilitate referrals and connections, and follow up to help ensure individuals are able to access the support they need. The role requires strong communication, community awareness, cultural responsiveness, and a genuine commitment to helping people navigate available resources and services.

Surveyor

As a Surveyor, you will conduct in-person assessments and audits within members’ homes and community settings to help ensure services are delivered consistently, safely, and in accordance with program requirements. You will document observations, verify required information, identify potential gaps or concerns, and support quality assurance activities across SMCS programs.

The role involves working directly with members and program teams, completing standardized assessment and audit tools, maintaining accurate and timely documentation, and communicating findings to the appropriate internal teams. Surveyors help identify opportunities for improvement and ensure that program activities align with established standards, contractual requirements, and regulatory expectations.
